Mountain Park Health Center Foundation

MPHC_FoundationAt Mountain Park Health Center, we understand the healthcare needs of our community go beyond providing basic medical care — whether that means providing transportation to medical appointments, access to medications or free mammograms.

The Mountain Park Health Center Foundation helps ensure that these important services are always available to those who need them, and supports our mission in a variety of ways — from fundraising and increasing awareness for Mountain Park to special community healthcare projects.

Your Support Makes a Difference

Your donations are essential to our success. Every donation to the Foundation helps ensure that we can continue to have the resources, staffing and facilities necessary to provide access to affordable healthcare for families in our community.

You can help provide healthcare for families in our community and ensure that quality community healthcare will be here for those who need it. The Arizona Working Poor Tax Credit program provides a dollar-for-dollar credit (up to $200 for single taxpayers, $400 for married taxpayers) if you itemize deductions on your state income taxes.

Here’s How Your Donation Helped

The Mountain Park Health Center Foundation works to make a difference in the lives of our patients. Here are a just a few of the ways we help:

Transportation to cancer treatment appointments

For women diagnosed with breast cancer, taking the bus to chemotherapy or radiation appointments is not ideal. Women often tell us they might not get to these appointments if not for the transportation vouchers they receive from the Mountain Park Foundation.

Medication Assistance

For some of our patients, the cost of a medication can be so prohibitive that they simply stop taking the drug — which can be life-threatening in some situations. Our Pharmacy Assistance Fund helps patients applying for pharmaceutical drug assistance programs or AHCCCS to stay on their needed medication during the application process.

Low Cost Mammograms

The Foundation helps provide free mammograms for women who couldn’t otherwise afford it.

Healthy lifestyle classes for teens/adults diagnosed as obese

The Foundation, through a partnership with the Arizona Bridge to Independent Living (ABIL), provide funds  which allow eligible high-risk adolescents membership to the ABIL gym facility. Once there the group meets weekly, attends trainer-led work out sessions and meets with Mountain Park nutritionist and behavioral health professional to help address both mental and physical health.

Thanks to the support of the community, Mountain Park Health Center was able to continue its mission of providing excellent, affordable healthcare in 2011. We served more than 50,000 people, providing preventive care, mammograms, vaccinations for kids, health education and more. View the 2011 annual report.
